    When enrolling in the GA-MTH program for the first time, use this form or register online.

    GA-MTH Renewal Form

    BEFORE a designation expires, renew by completing 8 hours of approved continuing education and then submit a Renewal Application. (Note: Required hours reduced as of Feb. 2019). Refer to the renewal form instructions page for details on continuing education requirements, fees and our mailing address. DO NOT USE THIS FORM IF YOU HAVE EXPIRED.

    GA-MTH Reinstatement Form

    AFTER a designation expires, Reinstatement rather than Renewal is required to re-activate your designation. The required number of continuing education hours increase for each year of expiration. Refer to the reinstatement form instructions page for more detail on continuing education requirements, fee, and mailing address.

    This form is used to order a replacement for a lost GA-MTH card.

    This form is used to apply for transfer from another SFI-recognized logger education program into the GA-MTH program.
